今日は、プレテストで予習していたのですが、この問題は答えを見てもどうもわからない

答えはｄだと思ったんですが、正解はｂだそうです
お年寄りだから、脱水を疑えということでしょうか？心房細動はどうなっちゃうんでしょうね？

私も今年ACLSを取ったばかりなので、この問題まだ記憶に新しいです。
最初の問題は、消去法で、aとdの答えはAMIの「典型的」な症状がでるケースではないでしょうか。 bも、年齢（56歳）と一日3パックの禁煙ということで、AMIの可能性大。 cのI型糖尿病歴22年ということで、糖尿による血管のダメージなどで病気の進行がmaskされやすく、tisaneさんのおっしゃるように典型的なAMIの症状が出にくいのでは？
次の問題は、ACLSのクラスでもレビューがありましたが、A-fibでも血圧が安定していて、症状も軽いことから、まずは酸素と水分補給、という答えになるということだったと思います。 ちょっと自信ないですけど。。
どんなテストでも、"most appropriate" とか"next"とかのキーワードが微妙が引っ掛けになるんですよね。
答えの理由がわかったらぜひ教えてください。 私も知りたいです。
